We request that all prospective patients read the information below before registering
At your first appointment, a careful and complete periodontal examination will be performed. During this examination Dr. Bouclin and his team will:
- Review your in-depth medical and dental history;
- Measure your oral health, by measuring and evaluating bone loss, loose teeth, bite, and other signs of periodontal disease;
- Perform an oral cancer screen;
- Perform a full muscle and jaw joint evaluation;
- Take digital photographs;
- Take only necessary x-rays ;
- Take only necessary impressions for study models.
After gathering all necessary diagnostic information, Dr. Bouclin will evaluate his findings and discuss any dental health issues that may have been uncovered by this examination.
At the end of the session, Dr. Bouclin will present the treatment options available including the necessary fees. He will work with you to customize a staged-treatment plan that will help you reach your health goals.
In some cases, the treatment plan will require coordinated care with your family dentist or another specialist.
Treatment is not traditionally rendered at the initial appointment. The consultation is intended to gain information, listen to your questions and concerns and to help provide solutions leading you to better health.
Occasionally, treatment can be performed the same day as the consultation.
IMPORTANT: For patients Under 16
For the purposes of legal consent and authorization to treat, a parent or legal guardian must be present at a consultation involving minors, and will be required to sign patient records.
